Will private health plans such as my employer sponsored health plan pay for CardioChek cholesterol check products? Are HMO’s currently paying for CardioChek cholesterol check products? There is evidence that some employer sponsored health plans are paying for CardioChek cholesterol check products including lipid (blood fat) tests.  Please check with your particular health plan.  You may wish to gain precertification from your health plan before purchasing CardioChek cholesterol check products.

Are HMO’s currently paying for CardioChek cholesterol check products?

At this time, most HMO’s will not pay for CardioChek cholesterol check products.  However, in a few cases, a HMO may pay for the CardioChek system as a glucose meter if your doctor says you need one.  Please check with your particular HMO.  If an HMO does pay, expect only partial payment for the CardioChek system, and payment for only diabetes related supplies. 

Do I need a prescription from my doctor to purchase CardioChek products?

CardioChek products are available over-the-counter.  This means you can buy it at a retail store such as a pharmacy or direct from Polymer Technology Systems, Inc., the makers of the CardioChek.  If you do not intend to file an insurance claim, you do not have to prove you medically need CardioChek products.

Do I need a prescription from my doctor to file an insurance claim for CardioChek cholesterol check products? 

Yes.  Everyone who is seeking payment or reimbursement from their health plan, including Medicare and Medicaid, needs a prescription from their doctor for the purchase of CardioChek cholesterol check products.  The prescription will be requested by your health plan.  

If I purchase CardioChek products direct from Polymer Technology Systems, Inc., or from a television shopping show, can I file a claim to my health plan?

In these cases, you may be able to gain reimbursement for CardioChek products if you file a claim to your private health plan.  Submit your CardioChek product purchase invoice and your doctor’s prescription for the products to your private health plan.  Medicare or Medicaid does not usually accept claims submitted directly by the consumer.
